Subleasing your unit means that you have found someone to take over your lease for a period of time. However during this period, you remain responsible for the subtenants actions and all other requirements stipulated in your lease, including rent payments.

A Lease Transfer removes you from all responsibilities stipulated in your lease and places the new tenant responsible for these responsibilities moving forward.
